Office Policies



New patients should fill out patient forms ahead of time if possible and please bring a copy of their insurance card and drivers license for the visit. Whenever possible, old records should be faxed to our office (314) 997-5086. Please bring your medications with you.


We have a 24 hour cancellation policy. If you are unable to keep you appointment, please try to give 24 hour notice. If you do not show up for your appointment, you will be charged a no-show fee. Please call 314-997-0554 at least 24 hours prior to your appointment to avoid a late cancellation fee.


Insurance regulations require payment of these encounter fees at the time of service.


If you have a true medical emergency you should call 911. If the problem is urgent, a telephone call to the physician with that statement is best so that the appropriate triage can be performed.

Insurance and Referral

Please obtain HMO referrals well in advance of the appointment. We will file insurance claims for you for our services.

Prescription Refills

Please call or have the pharmacist call before 2:00 PM for routine refills.

Telephone Calls

Routine telephone calls should be made between 9:00 AM and 4:45 PM Monday through Friday. Emergency telephone calls after office hours should be directed to the exchange (314) 388-6578. True medical emergencies that can not wait should prompt a 911 call. Return telephone calls by physicians will be made throughout the course of the day – please tell us how long you will be at your telephone number and how urgent the problem is. We perform endoscopic procedures most mornings and return calls in between cases or at the end of the work day for elective calls.
